On File Reference screen, Under header Shipper/Consignee/Notify party information, create a new sub section n" Notify 1" . The sub section should have the following fields:

Individual / Company ( Backend: Client type)
Name (Text field)

Phone (Text field)
Email Address (Text field)
Address (Text field)
Address 2 (Text field)
Address 3 (Text field)
