Please create a Credit Form for MCL & CP World according to the format provided below. 

 The form should be available if a client is tagged as a credit client, be it during client creation or updating an existing client's payment terms. 

 The user should be able to **download** download the credit form from the client maintenance screen and should also have the functionality to **upload** upload the signed document.
